Missing Notifications

I keep logging in, and some of my notifications don't pop up, and I've cleared my cache. But whenever I'm just lounging about, and I check out threads, sometimes no notifications come.
This has happened with pings, and friend requests. Is it just me?

Yeah, I was pinged by someone who wanted to friend me today, and I found two more friend requests.
It was weird!
I didn't even get a notification for any of them. Just that one.
